How to submit this form

Send the completed form by email to karol@consultad.io or by post to the address above. You do not have to use this form -- any clear statement of your decision to withdraw is sufficient. The withdrawal period expires fourteen (14) days after the date of subscribing.

Please note: by activating a paid Plan and starting to use the paid features, you expressly request that the service begins immediately and you acknowledge that you lose the right of withdrawal once the service has been fully performed in respect of any billing period during which you have used the paid features (see Terms of Service, section 4.8).
